Your Gateway Medicard keeps your personal medical profile handy at all times. Carry it with you at wherever you are - at home, work, and even on vacation. You'II feel more secure knowing that emergency medical personnel will have access to the data needed to give immediate care. When you send in your Personal Medical Profile, it is photographed on microfilm and laminated on to a durable plastic card. Only your Personal Medical Profile will be copied onto your card. Separate papers or other medical forms cannot be accepted. Be sure all pertinent information appears on the Personal Medical Profile. As a member, you may order one free medical card per account each year. It's important that you update your card annually to ensure that your data is current.
