How to use the Rx Discount program:

1. Present your USAccess card to a participating pharmacy. If you need assistance in locating a participating pharmacy in your area, contact a Member Services Representative at 1-800-308-0374.
2. Once verified, you will be eligible for the negotiated price for your prescription.
3. Pay for your discounted prescription. Your actual cost will be the predetermined contract price or the pharmacy's usual and customary charge that day, whichever is lower.

How to use the Mail Order Rx Discount program:

1. To receive a free price quote on mail order prescription drugs, call 1-800-396-2219. You will need to provide your member number and Rx ID number printed on the front of your USAccess card.
   
2. To obtain a mail order form, click on www.savrx.com or contact a Member Services Representative at 1-800-308-0374.
   
3. Mail the order form and your prescription directly to:

3. Select the eyewear of your choice. Most frames, lenses and specialty items such as tints, coatings and UV protection are eligible for discounts.

How to use the Mail Order Contact Lens Service:

1. For a price quote on your brand or type of lens call 1-800-878-3901. Most major brands of soft contact lenses are available including disposable, torics, and bifocals. Gas permeable lenses are also available.
2. Mail a valid contact lens prescription along with your member number printed on the front of your USAccess card to:

How To Use The Mail Order Non-Prescription Sunglasses Service:

2. Place your order. Most orders are shipped within 24 hours and can be paid for by check, money order, Visa or MasterCard.

3. Pay for your discounted medical services. If the provider's fee for services is less than $1000, you will pay the reduced rate upon completion of your visit. If the fee is $1000 or greater, you will receive a billing statement detailing your savings and the amount due for services. This amount is to be paid directly to the provider within 30 days of services.

NOTE: This is not an insurance plan and there will be no reimbursement to the member or the participating provider. USAccess members are fully responsible for paying the participating providers directly for services rendered.

AIR AMBULANCE

Suppose you were involved in an accident miles away from a medical facility equipped and staffed to treat your case? Unexpected emergencies like these take place every day, and dealing with them is extremely stressful and complicated. The solution is Network Air Medical Systems International Inc. (NAMS International), a unique service providing air medical transport within the contiguous United States, and within Alaska and Hawaii.

Following a medical emergency, a medically equipped and staffed air ambulance can rush you or a family member to an appropriate medical center when determined medically necessary by your attending physician and the NAMS Medical Director. NAMS International was selected as a USAccess benefit so its members would never be alone in a life-threatening situation.

In addition to air medical transport, NAMS service provides the following services in conjunction with a covered medical event:

• Transportation for a companion to accompany the covered patient
• Transport of organs for transplant procedures
• Transportation for a member of the patient's family to attend to minor children left unattended more than 100 miles from home due to parental accident/illness
• Vehicle return service if the patient is transported by air to a medical facility as a result of an out-of-town accident/illness
• Return of mortal remains if death occurs more than 100 miles from home
